• We welcome new patients who live within the practice boundary, please contact the practice for further details as we are awaiting a new map to be drawn.
  • Before joining the practice all new patients will be asked to fill in a medical questionnaire, provide their NHS number, proof of address and bring an up-to-date photo identification. New patients requiring repeat medication will need to see a doctor to have this added to their record.

Controlled Drugs

Due to the risks of addiction and current guidelines regarding adverse health outcomes will not be continuing prescriptions of the following medication.

  • Gabapentinoids –Gabapentin, Neurontin, Pregabalin, Lyrica, Alzain, Axalid
  • Opiates –Morphine, MST, Zomorph, Oxycodone, Longtec, Shortec, Oxycontin, Tramadol, Buprenorphine, Temgesic, Butrans, Reletrans, Hapoctasin, Butec, Transtec, Fentanyl, Durogesic, Matrifen, Tapentadol, Palexia
  • Hypnotics/Anxiolytics/Benzodiazepines –Diazepam, Temazepam, Nitrazepam, Lorazepam, Oxazepam, Chlordiazepoxide, Zopiclone, Zolpidem

You will need to agree to a reducing regime for your own personal safety and sign a form agreeing to this.

Your registration will actually be with the surgery but on your registration form you will be able to state your preference for a particular doctor.

Patients moving outside the practice boundary will need to register at another surgery.
