Some services we provide are not covered under our contract with the NHS and therefore attract charges.
Examples include the following:
- Claim forms
- Photocopying/Self-Certificate
- Fire Arm Licence
- Holiday Cancellation Insurance Claim
- Course of Injections (Vaccine costs are additional)
- HGV Medical
- Taxi Medical (New)
- Taxi Medical (Renewal)

- In the first instance you will need to complete a request form, which can obtained from Reception or you can print the form off and email this to [email protected]
