NON-NHS SERVICES
Some services we provide are not covered under our contract with the NHS and therefore attract charges. Examples include the following:-
- Simple Certificate/Claim Form/Letter
- Pip Supporting Letter (at doctors’ discretion)
- Photocopying/Self-Certifcate
- Holiday Cancellation Insurance Claim
- Course of Injections (Vaccine costs are additional)

In the first instance you will need to complete a request form, which can obtained from Reception or you can print the form off and email this to reception.w98003@wales.nhs.uk
