Register with us as a New Patient
We welcome new patients who live within the practice boundary. Please contact the practice for further details, as we are awaiting a new map to be drawn.
Your registration will be with the surgery. On your registration form, you will be able to state your preference for a particular doctor.
Patients moving outside the practice boundary will need to register at another surgery.
Before joining the practice
Before joining the practice, all new patients will be asked to:
- Fill in a medical questionnaire
- Provide their NHS number
- Provide proof of address
- Bring up-to-date photo identification
New patients requiring repeat medication will need to see a doctor to have this added to their record.
