Non-NHS Services
Some services we provide are not covered under our NHS contract and therefore attract a charge.
These are usually private services, such as letters, forms, medical reports or medical assessments requested for non-NHS purposes.
Examples of non-NHS services
Examples may include:
- claim forms
- photocopying or self-certificates
- firearms licence reports
- holiday cancellation insurance claims
- courses of injections, where vaccine costs are additional
- HGV medicals
- taxi medicals for new applications
- taxi medicals for renewals

How to make a request
In the first instance, you will need to complete a request form.
You can obtain a form from reception, or you can print the form and email it to:
