Change of Personal Details

If you change your name, address or telephone number please come in to complete, and sign, a “Change of Details” form as soon as possible or alternatively print a copy out from the link below and return it to Cwmelin Medical Centre. Where appropriate please bring the associated certificate and or proof of address which we will copy and return to you.

Unfortunately we cannot change these details over the telephone.

If you move without telling us we will waste time by visiting, writing to or telephoning the wrong house or number. It may also cause delays in passing important clinical information to you. It is particularly important that we have your up-to-date telephone number(s), especially as we are now using a patient text messaging service.

Remember to tell other agencies about your change of address too, for example your dentist, and if you have young children to will need to inform Central Clinic.
