How to Register

We welcome new patients who live within the practice boundary. 

Before joining the practice all new patients will be asked to fill in a medical questionnaire. New patients requiring repeat medication will need to see a doctor to have this added to their record.

We only accept new registration forms from patients who provide us with a photograph ID, proof of address and an NHS number.

Your registration will actually be with the surgery but on your registration form you will be able to state your preference for a particular doctor.

Patients moving outside the practice boundary will need to register at another surgery.
